We accept most traditional PPO plans and AHCCCS plans, including:

Aetna, Ameritas, Assurant, Banner, Blue Cross / Blue Shield, Cigna, Delta Dental, Guardian, GEHA,  Humana, Metlife, Premier Access, Principal, Prudential, United Health Care, United Concordia, Total Dental Administrators,  APIPA, APIPA Dual Complete, and CMDP

*We do not accept HMO and DHMO plans, outside discount plans, MercyCare, Care1st , Maricopa Health Plan, and Phoenix Health Plan. 

Financing Options at Felhake & Associates – Discounts on Dental Services

For patients without insurance or those with an unsupported insurance plan, we offer our own in-house discount plan.  The Show Me That Smile Discount Plan includes benefits and cost listed below. 

• 80% off two cleanings per year
• 80% off two periodic exams per year
• 80% off all necessary x-rays per year

On an annual basis, participants also receive:

• 25% discount on all dental treatments and procedures
       *Excluding Invisalign® and whitening
• 25% discount on additional cleaning

Pricing for our affordable discount programs is as follows:

Adult Program: $149.00 for 1-year contract paid in full at start of contract period

Child Program: $129.00 for 1-year contract paid in full at start of contract period
